500 Milliliters of /vr/ is a community add-on by members of the /vr/ board on 4chan released on February 1, 2021. It features six levels created by three authors, and features numerous replaced assets. Four levels are available for Bloodbath. It is named as a play on the 200 Minutes of /vr/ and related community projects for Doom.
A three level long episode for Blood by Swedish mapper Christian Green, a member of Team BANZAI, who spent awhile creating the maps; maps date from fall 1997 and were first released on Game Leader, and were re-shared from The Krunatus Blood Page from August 7, 2003.
Caleb finds an axe zombie carelessly wandering around a cliff-face, falling to its death. Caleb figures this must herald a nearby Cabal stronghold, and so he decides to check it out, again to relieve his endless boredom. First he comes across an old library, which he blasts through, before retreating down into the sewers, and again through a laboratory, finding a similar portal that leads out into a lava temple. There he has a surprise encounter with Tchernobog, whom he defeats before once again heading along his way.
Caleb has been hiding out in his cabin for a long time, getting increasingly bored, but weary of all the dangers that lay beyond. After all "it's a war out there". Eventually however, the protagonist decides to venture outward, figuring that all those things that seek to hurt him he can hurt in return, and thereby find his means to amuse himself. The level features cemetery, urban, library, sewer, water, and cave areas and even a laboratory with portals to both the frozen north and to a temple complex. It is here that the player meets his final challenge: two beasts that guard the exit.
Time Episode is an eight level fan add-on for Blood created by Pyro on September 17, 1999. The player is a mass murderer who in a fit of remorse, or because the authorities have finally caught up to him, leaps off a tall building to end it all, and is sent to hell. From thereon out it is his quest to escape hell through a time gate and then make it back to his own time.

The Way of Ira is a fan add-on for Blood by MariusArmand currently featuring two chapters, the first released on September 22, 2019 and the second on July 9, 2021. The title is derived from "ira", the Latin word for "rage" (or "ire"). The add-on features narration and new Caleb dialogue performed by David "Tekedon" Wikström, as well as tracks from the Blood Music Project.
Front Line is a vertically scrolling action game. Your mission is to infiltrate enemy territory and destroy their fortress. To reach the fortress, you will have to make your way through varied and dangerous terrain. Jungles, deserts, brush, and rocks all slow your progress, plus each area has numerous enemy fighters and tanks trying to stop you. To help get past these obstacles, you are armed with a machine gun and grenades; at some points in the game you may even come across an abandoned tank which you can control to increase your odds of survival. When you reach the end of the level and successfully destroy the fortress, the game will repeat at a higher level of difficulty. Gameplay is for one or two players, and four different skill levels are available.
Caleb Will Have His Revenge on Toronto is a Blood fan add-on by members of the Russian Blood Community which released its first episode, "Scum of Dallas", on July 29, 2013. A sequel, "Run Like a Villain", was begun, but is now not certain to ever be finished. This includes new animated cutscenes, having the Flag of Canada as the menu background, and replacing the Sawed-Off Shotgun and Aerosol Can with their darker versions from the game's development.
