Unlike its predecessor, Capcom vs. SNK 2, this game lacks both air guards and a "groove system", instead focusing on quicker gameplay. Characters are provided with a basic 3-level bar system for executing super-special attacks with basic filling options attributed to strikes and damage. Additionally, the sprites from the various series received new models and some special effects were changed in the command lists (for example, the player cannot increase the damage of the Zujou Sashi used by Choi Bounge by tapping the buttons anymore).

    Real Bout changes the play controls from the previous Fatal Fury games, reducing the number of attack buttons from four to three: a standard Punch and Kick button, a "Strong Attack" button which can be either a stronger punch or kick attack, depending on the character. The game retains the three-plane "oversway" system from Fatal Fury 3, which features a main lane for fighting, with foreground and background planes used to avoid attacks or leap towards the opponent. A dedicated button is now used to make an "oversway" (or change plane) towards the background or foreground. Real Bout introduces a Power Gauge, which fills up as the player performs normal or special techniques against their opponent or defend themselves, similar to many super move gauges featured in other fighting games. The Power Gauge allows players to perform one of three types of Special Techniques, depending of the level of the Power Gauge. Real Bout also introduces stages with ring-outs, a gameplay feature previously introduced in 3D fighting

The two new characters were the Judoka-assassin, Goh Hinogami and the Muay Thai boxer Brad Burns. With the addition of Brad Burns, Vanessa Lewis's alternate Muay Thai style from Virtua Fighter 4 was removed and replaced with a Vale Tudo "Offensive" style; making her a solely Vale Tudo brawler. All of the stages were adjusted. For example, Lei-Fei's stage in Virtua Fighter 4 previously took place during sunset, in Evolution it takes place during the morning.
